During the investigation of every incident, users are prompted to answer a series of questions. This process creates an automatic answer that provides an insight into the root cause of the incident such as Error, Recklessness or Situational Violation.
The Root Cause outcomes can then be acted upon and measures put in place to reduce the risk of the same type of incident occurring again in the future. As with the main Incident Management application, the Root Cause analysis is fully auditable, allowing the full analysis of the investigation process.
